Please note – you would have to download the consent forms and sign them on your own device and then EMAIL THEM BACK to us at reception@vitalab.com for Sandton or kznreception@vitalabkzn.com for Durban. There is an online consent form option available for Sandton patients for some of the forms. If you fill it in online and submit- you will receieve an online copy of the form and you do not have to worry about emailing us the forms back. Thank you!
